What Happens If Intimate Dryness Goes Untreated? A Gynaecologist Explains

First, let's talk about what intimate dryness actually is

Intimate dryness (a blanket term for vulval and/or vaginal dryness) isn't a weakness or something you've caused. It's a physiological response — most commonly linked to falling oestrogen levels. When oestrogen declines (during perimenopause, menopause, postpartum, or even during certain phases of your cycle), the vaginal walls thin and produce less natural lubrication. The result? Dryness, sensitivity, and discomfort that can affect everything from your daily comfort to your sex life.

And it is far more common than the world would have you believe. According to Dr Minhas, intimate dryness affects the majority of women at some point in their lives — yet it remains dramatically undertreated.

The Vaginal Microbiome

Before we get into what happens when dryness is left unaddressed, here's something your biology class probably didn't cover: your vaginal microbiome.

"The vaginal microbiome is an ecosystem of bacteria that lives in and around the vagina, playing a crucial role in protecting your intimate health. In a balanced state, it's largely dominated by Lactobacillus bacteria, which maintain an acidic pH that keeps harmful bacteria and infections at bay.

When oestrogen drops, this balance shifts. The vaginal lining becomes less hospitable to Lactobacillus, the pH rises, and the microbiome becomes more vulnerable to disruption."

As Dr Minhas explains, this creates a cascade effect — dryness and microbiome imbalance feed into each other, increasing susceptibility to recurrent infections, irritation, and further discomfort.

Vaginal dryness in menopause

Of all the stages where vaginal dryness becomes a persistent issue, menopause is where it's most significant — and most consistently ignored.

Vaginal dryness in menopause is part of a wider condition called Genitourinary Syndrome of Menopause (GSM). Unlike hot flushes or brain fog, which often ease over time, GSM tends to worsen without treatment. Dr Minhas is clear on this: vaginal dryness in menopause is a progressive condition. The tissue changes don't reverse on their own.

Left untreated, the symptoms can include:

  • Chronic discomfort and pain, including during sex (dyspareunia), which affects relationships, self-esteem and quality of life
  • Recurrent UTIs and bacterial infections, as a disrupted vaginal microbiome leaves you more vulnerable
  • Vaginal atrophy, where the vaginal walls thin further, leading to tearing, bleeding and increased inflammation
  • Urinary symptoms, including urgency, frequency and incontinence — often dismissed as "just getting older"

The message is not to frighten you. It's to empower you. Because every one of these outcomes is manageable when you have the right information and the right support.

So, What Can You Do?

The good news — and there genuinely is plenty of it — is that intimate dryness responds well to targeted, consistent care.
